The main causes of accidents such as contact damages on departure are due to (1) insufficient preparations, (2) lack of local knowledge, (3) failure to post a proper lookout and (4) failure to properly operate Bridge Resource Management. Attached you will find our 23rd case study for onboard safety meetings which we have called “Lines in the water”. The Case studies for onboard safety meetings consist of a short text describing a common operation onboard most vessels, and a risk assessment form. The crew should read the text and try to establish the errors made by the crew in the text. We recommend that the risk assessment form is used as a template to discuss the story and carry out a risk assessment of the operation described. Details please refer to attached Circular.
